Doctor's Appointment

FAQ

  • What should I bring to my appointmant?
Please bring your intake form (ideally it should be filled out prior to your first session so you can maximize your time with your RD), along with your doctor's prescription for our services (if you have one), and copies of your blood tests and other lab work results. For pediatric appointments please also bring your child's weight and height history from your pediatrician.
  • I'm uncomfortable being weighed, is this a must?"
Prior to your first weigh in (if appropriate) please let our office staff know if you do not wish to know your weight and we can record your weight "blind" and not let you see it.
  • What are your fees?
Please call the office for our most updated fee schedule.
  • Do you take Insurance?
Like many RD offices, we do not bill insurance directly. Instead, we provide you with a superbill after you render payment on the day of service. We will give you instructions on how to best negotiate with your insurance provider for reimbursement. Your level of reimbursement will depend on your individual insurance plan and your medical diagnosis, and our staff is committed to helping you achieve maximum reimbursement in any way we can.
